What is the Partnership Date for my client?

Partnership Date for AB 2034 clients is either July 1, 2007 OR the first day services are provided after July 1, 2007

What do the various abbreviations mean? What are the equivalents?

PAF = Baseline

KET = KEC (Key Event Change)

Quarterly = 3M (Three Month Assessment)

What Referral Source should I use?

If you have information about the original Referral, that would be best. If not, use Mental Health Facility/Community Agency
