Adult KEC Living Arrangements

Page history last edited by PBworks 16 years, 2 months ago

The living arrangement section is completed when there is a change in a client's living arrangement.  When a client is hospitalized, a new KEC needs to be completed, to indicate the date of the change.  When the client is discharged from the hospital, another KEC will need to be completed to document the client's new living placement (i.e., board and care) thus indicating that the client is released from the hospital.

Client has had a change in living arrangement? (check one in this column)
