I understand that it is optional to complete the baseline outcome measures for existing FCCS clients. Now the question is if we choose not to go back and complete the outcome measures, at what point to we implement the outcome measures during treatment. Would we just not do any at all for these clients, or would we complete one at their first 6 month cycle (and would that be a baseline or 6 month that we would complete at their first 6 month)?
If you choose not to go back, prior to March 1st and do a baseline on the client when they started FCCS, you will not be able to collect outcomes on these clients at all. You must start any new clients after March 1st with a baseline. For clients already in your program as of March 1st, if you don't go back to do baseline lines and subsequent 6 month and 1 year updates, you will not be able to collect any data on these clients.
In your example, if you do not go back on a client that started in January, you will not be able to collect outcomes. If you feel you have enough data to go back to January and you will decide to do that for all your clients in your program as of March 1st, you would do the baseline measure and then the 6 month update would be due in July.
For Living Arrangements, what's the time period we are evaluating at 6 months or one year? Is it the whole 6 months leading up to the date, or is it the last 30 days?
Provide the most frequent (modal) residential type that your client stayed at within the 30 days leading up to the 6-month due date. In another words, if within 30 days prior to the review day your client stayed mostly with adult family members amongst other placements, you will indicate the adult family member placement as the primary residence at the time of the six month review.
What should we have therapists do with the Outcome Measures forms they have already filled out for clients since these forms do not have the End date on them? Should I instruct them to transfer the info they filled out from the old form to the new form, which has the end date and shred the old forms or should I have them use the new form on clients who are opened under FCCS from now on?
The bottom line is that you should do what makes sense for your staff and agency. You won't be sending those forms to us in the end: you will be entering them in the OMA FCCS database we are presently designing for you. So you should do what you need to do to make the forms clear to your data entry staff. What does that mean? If it were my agency and it's not too much trouble to transfer the information over, that would be ideal. But maybe staff could staple the old form to the new form and use the new form from now on for the updates (keeping the baseline info handy for reference). In the end: what's going to make the most sense to your data entry people?
What do I do with Outcomes when a client dies? Is there something special I need to do for Outcomes?
No, just an end of services update. In other words, file a final OMA update. There are other proccesses you might need to follow to comply with DMH procedures related to incident reports on the death, but for OMA you need to do a last update. |
