Special instructions for clients in SFC

 

 

There are currently two separate programs that are tracking Outcomes for clients in the OMA: Full Service Partnerships (FSP) and Specialized Foster Care (SFC). In addition, SFC has two different levels (Basic and Intensive), and SFC is using the OMA to track results for those two levels of care separately.

 

 

Multiple programs or levels using the OMA are causing problems for some users. We’ve created this list of scenarios to help you figure out how to get around these problems.

 

 

SCENARIO 1

 

 

“My client already has a Baseline at another provider, and I can’t add a Key Event Change (KEC) or Three Month Assessment (3M). That provider provides services under the same program as I do (in other words, we’re both FSP providers OR we’re both SFC Basic providers OR we’re both SFC Intensive providers).”

 

 

If you are not the agency that created the original Baseline for the client, you’re going to need to get the original agency to enter a Key Event Change that identifies you as the new agency in charge of the client. In effect, the original agency needs to “hand off” the client to you by filing a KEC.

 

You can identify that original agency by looking at the open episodes and/or by looking at the first page of the Baseline. If you don’t know how to contact the agency, you can use the DMH website to find their contact information, you can look them up in the provider directory on the Integrated System website, or you can contact your Service Area administration. If you are unable to contact the agency or if they are unwilling to file the KEC, please contact your program management (either FSP or SFC).

 

 

Please know that you will never be able to edit a Baseline that was not created by your agency. You will, however, be responsible for the subsequent 3Ms once the client is your responsibility. You will need to look at the Partnership Date on the original Baseline and base your own 3M schedule on that date.

 

 

 

SCENARIO 2

“My client has a Baseline at another agency, but they are not under the same program as I am. Examples: The original agency was a Specialized Foster Care provider, but my agency is an FSP; or the original agency was a Basic SFC provider, and my agency is an Intensive SFC provider.”

 

 

Remember: each of the three programs (FSP, SFC Basic, SFC Intensive) are trying to collect their own Outcomes. What that means is that when someone moves into a new program, a new Baseline has to be done. In addition, you will need to change the Program in the OMA (from FSP to SFC, or from SFC Basic to SFC Intensive, for example).

 

In the example above the client has “Specialized Foster Care – Basic Mental Health Services” on their Programs tab. To begin the process, their SFC provider has to file a Key Event Change to close out that set of assessments. When a client is going from one program to another, it’s important to have the new program go into effect with the new Baseline, so once the FSP provider is ready to start services, they need to file a Deletion/Change Request Form asking us to change the program to FSP-Child (or FSP-TAY) with an effective date that matches the new Partnership Date for FSP services. They can then open an Episode in the Integrated System and create a Baseline for FSP services in the Outcomes Measures Application.

Let’s look at the different possibilities under this scenario. Take a look at the three examples below to see how to handle these changes…

 

 

Example #1: Our client was being seen by an agency under the Specialized Foster Care Basic program, but now they are coming into our agency as an FSP client. The SFC agency will eventually need to do a final KEC as the client leaves their program, but our FSP agency will file a Deletion/Change Request to change their program from SFC Basic to FSP, and then open an episode on the client in the IS and finally create a whole new Baseline connected to that episode. From now on, all the assessments that we do (3M and KEC) will be associated with that new Baseline.

 

 

Example #2: Our client is moving from SFC Basic to SFC Intensive. Because of the change in levels between these two programs, the same rule applies. The agency providing Intensive services will need to ask DMH to change the program using the Deletion/Change Request, open an episode and then do a new Baseline connected to that new episode. Remember: the new assessments will need to be connected to the new Baseline.

 

 

Example #3: My agency has both Specialized Foster Care and Full Service Partnerships, and I have a client going from SFC to FSP inside our agency. In this case, the answer is the same as #2. You will need to file a Deletion/Change Request, open a new episode and do a new Baseline.

Other SFC Information

 

• The rules and requirements for Specialized Foster Care DMHOMA activities, with a few exceptions, are largely the same as those for MHSA Full Service Partnerships and all other DMH OMA activites.

 

• Specialized Foster Care - SFC Providers are required to use the DMHOMA Forms and Guidelines to complete measures for all clients receiving Basic SFC MHS and SFC Intensive In-Home Services.

 

• BASIC Service Providers: Completion of SFC DMHOMA requirements are as follows:

All cases opened after: Service Areas 6 and 7 - December 1, 2006 Service Area 1 - April 1, 2007

 

• INTENSIVE IN-HOME Service Providers: Completion of SFC DMCOMA is required for all cases opened after August 1, 2007.

 

• DMHOMA data collection is encouraged but not required to be completed for any SFC cases opened prior to these above specified starting dates.

 

• Basic SFC MHS and SFC Intensive In-Home Services includes and is limited to only those clients receiving EPSDT funding from the Fall 2005 Enhanced Specialized Foster Care Board Letter Joint Plan.

 

• Specilaized Foster Care Services do not include all other DMH EPSDT funded Services that are also are provided to DCFS involved children and youth but not funded by the Fall 2005 Board Letter. Consequently, those non SFC cases do not require the completion of SFC DMHOMA .

 

• "Hard Copies" of OMA Forms are part of the client's Personal Health Information - PHI, and need to be attached to the clients Medical Record for information and audit purposes.

 

• "Who completes the Baseline? We strongly recommend that the person who completes the client's DMH Initial Assessment documents also complete DMH OMA Baseline within the same period of time. The client/family/primary engagement process needs to achieve a level of rapport and detailed inquiry to support the most complete Baseline findings as is possible. Likewise they are best completed within first few weeks of the Mental Health Episode or shortly after the episode start date. Delaying the Baseline completion, or completeing it as a separate activity fromt the Initial Assessment activities, undermines the establishment of the most accurate baseline findings and duplicates work efforts.
