The 3 month or quarterly assessment needs to be completed in its entirety every three months starting with the enrollment/partnership date. The window you have for completing the 3 month assessment is 15 days before the three month date and 30 days after. For example, if the enrollment date is March 1st, the first quarterly report is due June 1st, but can be completed anytime between May 17th and July 1st. The next quarterly report would be due on September 1st.
