Adult Living Arrangements

  • Days in this section must add up to 365
  • The determining question is always, "Where did the person lay their head at 11:59pm each night?"
  • Dates should not overlap
  • If a person is discharged from a hospital on a Monday (for example), they will not lay their head there that night. Discharge dates will not therefore match up exactly with Living Arrangements.  For example: if a client was discharged on 1/1, the client is most likely to be discharged during the day on 1/1.  Therefore, the client's last night in the hospital would be on 12/31.

     To calculate the dates for the living arrangement section you can use the following spreadsheet to determine the begining and ending dates for a clients Living Arrangement Section.

This spreadsheet will also help with determining the date ranges for when the clients 3Ms are due. 

 

 

For each of the following RESIDENTIAL TYPE categories, you need to either enter the data or check the appropriate boxes corresponding to:

-the "from" and "to" dates for each occurrence and total number of days for that 1 occurrence;

-where the partner will be "tonight" (where will they will be at 11:59 PM tonight);

-where the partner was "yesterday" (as of 11:59PM the day BEFORE the partnership );

--and indicate whether the client ever resided in that residential type anytime in their life prior to a year before the partnership date. Check all that applies.  

 

Each possible living arrangement only appears once on the paper forms. In the computer application these different living arrangements are part of a drop-down menu, so you can enter different incidents of the same type of living arrangement very easily. On the forms, you will need to use the "blank" "overflow"  sheet seen below.
