How to gain access to the Outcomes Measures Application
In order to gain access to the Outcomes Measurement Application you need to establish an identity in the Integrated System, and if you work at a Contract Provider site (as opposed to a DMH Directly Operated Clinic) you will need to apply for a RSA SecurID card. If your reporting unit is completely new to the system, you will also need to file the Authorization to Sign CIOB Access Forms statement.

The turnaround time for new access is about one month. The unit that processes those forms at DMH has the following advice/information for new applicants:
Many forms are received daily and get processed in the order they are received. The procedure for troubleshooting access is to have your contact person call the CIOB Help Desk at 213-351-1335 in order to create a call tracking ticket. If the submit date is prior to the processing date, a fax number is given to fax the form for immediate processing. The form must be submitted with all required fields completed and with an authorized manager/designee signature. If you DO have access to the Integrated System, go to the Logon to OMA page
Our provider site is brand new: nobody here has access to the Integrated System or the Outcomes Measurement Application.
First, determine WHO at your site is authorized to sign off as a Program Head or other Authorized designee. You need to tell DMH who at your agency has the authority to create accounts, issue RSA SecurID cards, and so on. Your administrator may already have provided that information...if not, please file the Authorization to Sign CIOB Access Forms revised 3/06/15...here's a SAMPLE
I'm a County employee, a and I've just been hired or transferred to this FSP program, and I don't have access to the IS.
First, make sure that your clinic has a copy of your Confidentiality Oath on file! You don't need to file that document with the DMH CIOB, but you must sign it and your clinic must keep it on file. Print out a copy of the Confidentiality Oath, or take a look at a Sample before you sign it.
Next, file an Applications Access Form. This form must be completed in order to get you a logon identity and password for the Integrated System (IS). Once you have access to the IS you'll able to logon to the Outcomes Measurement Application.
Download the Applications Access Form and Applications Access Form Instructions. You might also need to take a look at the Applications Access Form Codes, and you will certainly need to look at the list of Integrated System Access Roles.
- This form must be signed by BOTH THE USER AND THE AUTHORIZED SUPERVISOR FOR YOUR PROGRAM (that's the person listed in the Authorization to Sign CIOB Access Form mentioned above in the "Our Program is Brand New" section).
- You MUST include all the ROLES you will need (right under City-State-Zip). Please list ALL of the Roles you think you'll need from the Integrated System Access Roles document: for example, "CLN01, CLN02, ADM01, ADM02, and RPTPROV" will allow most workers to access all the sections of the application they need.
- You MUST include a DMH Provider number.
Please refer to the Applications Access Form Instructions if you need more information about a specific field.
The #1 reason for delays is that people do not complete this form! Please take the time to make sure you have completed it correctly. One thing that can help is if you can see an example completed by someone else at your location.

You need to focus on two items: completing the Applications Access Form (that will get you a logon and password on the system), and applying for an RSA SecurID card. You will file both sets of paperwork at the same time, but let's look at the RSA SecurID card first.
Applying for an RSA SecurID Card
The RSA SecurID card is a small oval plastic token you can attach to your keychain. It's got a battery and a little computer chip in it that is constantly generating special passcode numbers that are known only to the computer you're logging into. Since it's registered to you, it's a sort of digital key that is unique to you.
At this point, the RSA SecurID card is provided without fee to users. However, if you lose the card you will need to pay for a replacement, and in the future the County may require contract providers to pay for them.
To obtain a card, first you need to fill out the SecurID Card Agreement for Acceptable Use and Confidentiality statement. Please note there is a Sample that shows you how to fill out the form.
You also need to file Downey Data Center Registration form, which also comes with a Sample. You'll need to send these forms in with the materials for system Access (listed in the next section)....
