Baseline Assessment


The Baseline or Baseline Assessment in completed once, when the partnership is first established. The Baseline Assessment should be completed at the very beginning of the partnership to insure that you capture the full measure of the benefits that are delivered.

 

The State of California uses a form called a PAF or Partnership Assessment Form to capture history/baseline data in the following areas: residential (includes hospitalization and incarceration), education, employment, sources of financial support,legal issues/designations, emergency intervention, health status, substance abuse, ADL/IADL (Older Adults only).

 

Los Angeles County Department of Mental Health collects history/baseline data on a similar set of domains. The data collection forms are unique for each of the four age groups (Child baseline, TAY baseline, Adult baseline, Older Adult baseline). This data is temporarily being collected using a paper and pencil system.
