Grade Level Information

Highest level of education attained

 

Check the box next to the highest grade level completed. Example if the client is currently in 7th grade, the highest grade level completed would be 6th grade, so you would check the box that corresponds to 6th grade.

 

School Attendance

 

This is meant to measure truancy, not excused absences such as medical problems.

 

Use the following definitions to report school attendance over the last 12 months and the client's current level of attendance.

 

Most of the time: attends school four or more days per week

Sometimes: attends school between three and four days per week

Infrequently: attends school between one and three days per week

 

School Performance

 

Rate client's school performance by using the following criteria:

 

Very Good is A- or better average (4.0 or above GPA)

Good is B- to B+ average (3.0-4.0 GPA)

Average is C- to C+ average (2.0-3.0 GPA)

Below Average is D- to D+ average (1.0-2.0 GPA)

Poor is F or Failing (0.0-1.0 GPA)

 

Suspensions and Expulsions

 

Indicate the number of suspensions and expulsions for the client over the last 12 months. Please note you are reporting occurrances and not number of days. For example if the client was suspended 3 times for a total of 9 days you would write down 3 next to suspensions.
