A hospital, nursing facility, or other institution of more than 16 beds that is primarily engaged in providing a diagnosis, treatment, or care of persons with mental diseases, including medical attention, nursing care, and releated services The consumer is living in the facility because of his/her PSYCHIATRIC healthcare needs. There may be co-occurring physical healhtcare needs, which is why the consumer may be placed in a SNF rather than an IMD or MHRC.

 

IMD Facilities in LA County include La Casa MHRC, Landmark Medical Center, Laurel Park Center, La Paz, Community Care Center, View Heights Convalescent, Meadowbrook Manor, Olive Vista Center, and Sierra Vista.
