Lives in an apartment or house alone / with spouse / partner/ minor children / other dependents / roommate - must hold lease or share in rent / mortgage

 

Any situation in a home, apartment or trailer with its own cooking facilities and bathroom. Consumers either own the house or hold their own lease or rental agreement with the owner. The building might be owned by a mental health agency but there are no services based in the apartment (Case managers may visit the resident in the apartment and provide services there). The apartment manager might be hired by a mental health agency but has no treatment responsibilities.

 

Roommates, room and boards.

Consumers hold their own lease or rental agreement with the owner. There are no treatment staff in residence although case managers or other staff may visit the resident in the residence.

 

Supported Housing.

A housing unit located in an apartment complex, an SRO, a single-family residence, or a private building in which consumers hold their own lease or rental agreement with the owner or with a not-for-profit organization acting as the master leaser. Residents may live independently or share sleeping quarters with other consumers. In some situations, cooking facilities and bathrooms may be shared. Some social/clinical services are based in the facility and are provided on site in private offices or common areas. (It is the presence of these on-site services that distinguish this category from categories 20 through 22 below). In nearly all cases, these services are VOLUNTARY. The consumer is in no way required to participate in these services as a condition of their continued stay in the housing.
