Socializes with others: If the client socializes with others then circle/ select yes.

 

Receives Spiritual support: If the client receives spiritual support, circle yes.

 

Develops and maintains friendships: If the client develops and maintains friendships then circle/select yes. It does not matter if the friendships are a positive or negative influence on the client.

 

Requires protection from abuse: If the client is vulnerable and requires protection from abuse, then circle yes. If the client is under the jurisdiction of the Department of Child and Family Services (DCFS)or Adult Protective Services (APS), circle yes.

 

Child/Youth has age appropriate, positive peer relationships? If the client engages in peer relationships that are positive in nature and appropriate for their developemental age, circle yes. If the client has a couple of positive peer relationships, but mostly negative peer relationships, you should still circle/select yes for this question.

 

Child/Youth has age appropriate involvement in family? Does the client interact with their family as you would expect given their age and development level? If so, select yes.

 

Child/Youth has supportive interactions/relationships with:

 

Is the family or significant other(s) involved in the child/youth's treatment? If the client has someone that is involved in treatment of the client (coming to appointments, encouraging client to continue with FSP, family sessions, administering medications, collaterals, etc.) then the response here is yes.

 

Child/Youth has access to at least one stable, supportive adult? If the client has at least one positive adult relationship available to them outside of the treatment team then the response to this question is yes.
