What’s the effective date to start collecting data for Wrap on the OMA forms?

The effective date is July 1, 2009 for all Tier II FSP Child and FSP TAY enrolled clients. 

FSP providers are supposed to complete outcomes for all FSP clients back to July 09.  This was discussed several times at our Wraparound providers meetings and we followed up with a memo to remind them that this needed to be done.
